Frequently Asked Questions - Home and Community Based Services for the Frail Elderly (HCBS/FE)

HCBS/FE services provide an alternative to nursing facility care. These services promote independence in the community setting and ensure residency in the least restrictive environment.

What type of services can I receive from the HCBS/FE program?

The HCBS/FE program offers nine different services to assist the customer in maintaining their independence. They are: Adult Day Care, Assistive Technology, Attendant Care Services (Level I and II), Nursing Evaluation Visit, Personal Emergency Response, Sleep Cycle Support, Medication Reminder, Oral Health Services and Wellness Monitoring. These services are based upon customer need, which has been determined by the customer and case manager.

What criteria is required to qualify for HCBS/FE services?

The following criteria is required to qualify: The customer must be 65 years of age or older, financially eligible for Medicaid as determined by SRS, meet the functional eligibility criteria, and chooses HCBS/FE services.

How much money can an individual keep and still qualify for HCBS/FE services?

SRS makes the determination on financial eligibility for the customer. The Economic and Employment Support (EES) Specialist will work with work the customer to determine their assets. Many factors are reviewed by the EES Specialist when making the financial eligibility determination.

How many hours of service can an individual have when they need HCBS/FE services?

The hours of service are based on the needs of the customer. Each Plan of Care that is developed by the case manager is individualized to meet the customer’s needs.

Whom do I call to get information when I need help?

An individual can call the case management entity of their choice, that covers their county, and request help in accessing services that they need. See the link below.

What is self-directed care?

Self-Direction is an option that allows the customer in need of attendant care services to live in the least restrictive environment and direct their attendant care services. These services are determined by the case manager and customer to be essential to the maintenance of their health and welfare. When the customer chooses to direct their care, they are responsible for multiple tasks: such as, the selection of the attendant, training, monitoring, and dismissal of the attendant.

Can my family be paid for helping me?

Under the option to self-direct their care, the customer can choose a family member to provide attendant care services to them. However, a family member cannot be a spouse or a person who has been appointed by the customer or the court to represent them (Acting on Behalf, Activated Durable Power of Attorney, and Guardian/Conservator).

Will HCBS/FE services pay for an individual who chooses to live in an Assisted Living Facility, Residential Health Care Facility, Home Plus or Boarding Care Home?

The HCBS/FE program will pay for services delivered to the customer in an Assisted Living Facility, Residential Health Care Facility, Home Plus or Boarding Care Home. The customer is responsible for paying for their room and raw food costs. HCBS/FE will pay for the direct services provided to the customer in accordance with the Plan of Care.
